There's Monsignor Peter Elliot's 'Ceremonies of the Modern Rite' and Fr
Dennis Michno's 'A Priest's Handbook' which, although not aimed specifically
at servers, are very concise and can both be adapted to local Western Rites.
There is nothing specifically for the CE, and I doubt for your neck o' the
woods either. Perhaps the good people on the Mystery Worshipper board on
Ship of Fools can be of some assistance.
The Guild of S Stephen (our RC equivalent) has a publication which is
perhaps appropriate for people who have never been to Mass who are
considering serving, but it is extremely basic stuff. I know that the Gild
of Clerks are also putting a guide together for serving traditional
English
ceremonial according to modern Western Rites.
Perhaps it's worth mailing the secretary.
